Pre – Operative Care
\nCare may vary depending on type of surgery, eg. major or minor and if the surgery is elective or emergency<\/p>\n
Reduction of anxiety
\nGive a warm personal welcome
\nIntroduce yourself, other team members and other patients by name
\nExplain ward layout and nurse call system
\nExplain procedures before starting them
\nEncourage patient to express fears and anxieties
\nEncourage patient to ask questions
\nGive appropriate answers
\nTake time to communicate with significant others<\/p>\n
Gain information
\nComplete admission documents
\nRecord patients weight
\nRecord baseline TPR BP
\nNote any significant medical history
\nPossible ECG
\nCarry out ward based urinalysis
\nCheck for any allergies, clearly record on notes and drug charts<\/p>\n
Patient education
\nEducation may help prevent some post – op complications
\nGiving information may help reduce post operative pain
\nTeach how pain will be controlled
\nTeach deep breathing exercises
\nTeach how to hold the wound to cough
\nTeach importance of foot and leg movements
\nInvolve MDT as appropriate<\/p>\n
Fasting
\nNil by mouth for 4 – 6 hours before operation<\/p>\n
Patient Identification
\nCheck out of ward – name, address, DOB
\nCheck identity bracelet
\nCheck site of operation is marked if appropriate
\nSend notes, X rays, blood results etc with patient<\/p>\n
Consent
\nPatient must sign a consent form, consent must be informed<\/p>\n
General measures
\nTED stockings and s\/c heparin
\nOperation site preparation
\nAsk patient to empty bowels and bladder
\nShower and gown
\nGive pre-medication

Tape or remove jewellery
\nRemove nail varnish and make up
\nRemove prostheses spectacles\/contact lenses and hearing aid
\nNote if dentures are in situ and any loose teeth or crowns, (all late removal)
\nEnsure any specific preparation is carried out eg. bowel preparation
